 An Important Message from Nurse Trina

 Dear Parents/Guardians:

 Student safety is a priority at Hampton School; team effort and open communication is key in achieving any goal.

 When a student sustains an injury or suffers an acute exacerbation of an existing or newly diagnosed condition notification from the parent/guardian and receipt of medical documentation from the physician is critical.

 Parents/guardians should address medical issues and concerns with the school nurse. Medical documentation should be brought to the school nurse upon the student’s arrival at school.

 I’d like to take this opportunity to review the following procedure with you. It is from our School Physician and must be strictly adhered to:

 Any student requiring medical treatment for an injury will be required to furnish medical proof, from the treating physician, of ability to return to school and that they may participate in gym/physical education classes and any team sport that they are members of. Otherwise, they will be excluded from participation.
